Information Subpoena from a collection agency 

 

I received a letter today stating that I needed to answer the attached questionnaire with questions regarding my bank acct, social security number, place of employment, etcl The letter also stated that if I did NOT answer the questions, I would be arrested.  My question to you is this. Do i legally have to answer the questions? What can I do on my end to terminate further action against me? I'm a single mother of three children making a minimal salary and can not afford for this company to garnish my wages or levy my bank account as they have tried to do in the past. Please send me any advice that you have on this action. With much appreciation!!
