order to submit 

I have taken paperwork to ask the judge to overturn his decree of divorce by default that my wife was granted, I have now been told i did not hand in everything i needed I also need to had in an "Order to Submit" where do i find a form for this , i am not even sure what this is and what i need to include - can you please halp me with the basic form

it is very easy to get an example form "order to submit" from the judge's chambers.  Use your charm to persuade the JA (judicial assistant) to give you one.
